We are excited to have you join Sparks Florist as a Temporary Floral Designer. This form serves as your official offer letter. Please review the terms below carefully before signing.
1. Assignment & Schedule
- Position: Temporary Floral Designer
- Dates: This is a temporary assignment beginning on your first scheduled shift and ending upon the completion of your shift on February 14, 2026.
- Pay Rate: $14.00 per hour.
- Pay Method: Wages are paid via Direct Deposit according to the standard company payroll schedule.
2. Mandatory Onboarding Requirements
To finalize your hiring, you must complete the following steps before your first shift:
- Personal Information & Document Upload (Right Now): Upload clear photos of your Driver's License in the fields below.
-Policy & Procedure Review and Acknowledgement (Right Now): Review the policies & procedures included in this form. Once review, sign the acknowledgement at the end of the form.
-Training (Before First Day): Before your first shift, please watch the videos that will be sent to you after you have completed onboarding to learn about our arrangements.
- I-9 Verification (CRITICAL ON FIRST DAY): Because you are a new hire, federal law requires us to physically inspect your ID documents.
NOTE: On your first day, you MUST bring your PHYSICAL documents for your I-9 verification. Examples: A Valid U.S. Passport OR a Driver's License PLUS your Social Security Card (or Birth Certificate). Photos or copies are not accepted for this specific federal step. A list of accepted documents are included in this form.
