Sparks Florist Onboarding - 2026 Temporary Floral Delivery Driver - New Hire

Welcome to the Team! We are excited to have you join Sparks Florist as a Temporary Floral Delivery Driver. This form serves as your official offer letter. Please review the terms below carefully before signing.

1. Assignment & Schedule - Position: Temporary Floral Delivery Driver - Dates: This is a temporary assignment for February 14, 2026 only. - Pay Rate: $15.00 per hour. - Pay Method: Wages are paid via Direct Deposit according to the standard company payroll schedule.

2. Vehicle Use & Daily Bonus Policy Please read this section carefully. This policy determines if you get the extra $50. - If Driving a Company Vehicle: You receive the base rate of $15.00 per hour. You are NOT eligible for the daily vehicle allowance. - If Driving Your Personal Vehicle: You receive the base rate of $15.00 per hour PLUS $50.00 per day vehicle allowance.

To qualify for the $50.00 daily allowance, you must: 1. Complete your full scheduled shift. 2. Check out with Dispatch: You must receive verbal clearance that you are "released" for the day before leaving. Note: Failure to complete your route or leaving before being released by a supervisor will result in the forfeiture of the $50.00 allowance for that day.

Mandatory Onboarding Requirements To finalize your hiring, you must complete the following steps before your first shift: - Driver Information & Document Upload (Right Now): Upload clear photos of your Driver's License, Vehicle Registration, and Proof of Insurance in the fields below. -Policy & Procedure Review and Acknowledgement (Right Now): Review the policies & procedures included in this form. Once review, sign the acknowledgement at the end of the form.

-Training (Before First Day): Before your first shift, you must download the "Trega" App and watch the instruction video. We will email you more details, including your login information, once we have received the information on this form and entered you into our system.

- I-9 Verification (CRITICAL ON FIRST DAY): Because you are a new hire, federal law requires us to physically inspect your ID documents. NOTE: On your first day, you MUST bring your PHYSICAL documents for your I-9 verification. Examples: A Valid U.S. Passport OR a Driver's License PLUS your Social Security Card (or Birth Certificate). Photos or copies are not accepted for this specific federal step. A list of accepted documents are included in this form.
